As a Basic 245D licensed provider of Personal Supports, Homemaker, Adult Companion and Night Supervision Services, Comfort Services LLC will work with you and your county case manager to administer your Basic245D licensed services. Comfort Services LLC will meet with you initially, and annually, to complete a plan of care, as well as, ensure all workers are properly trained when they are hired. Basic245D services are available under the Minnesota home and community-based waivered programs, including the Developmental Disabilities (DD) and the Community Access for Disability Inclusion (CADI) Waivers.
Personal Supports is non-medical care, assistance and supervision provided in the individual’s home or community. This one-on-one service is intended to increase one’s independence and involvement with community activities. Personal Support Services are available to individuals on either the Developmental Disabilities (DD) Waiver, Community Alternative Care (CAC) Waiver or the Community Access for Disability Inclusion (CADI) Waiver.
